What is a Form 11 and why is it important?
The "Form 11" is how Washington DeMolay keeps track of who is in charge in each chapter. We use this information to update our State Directory so the State Corps and Executive Officer can contact you about new events or programs that are happening, ask you how things are doing in your Chapter, and mail you various DeMolay advertisements and promotions. Washington DeMolay believes in putting chapters first, and having an up-to-date means of communications with your Chapter enables us to do so.

The form requires many items to be filled in because we want as much info as possible. IF you don't have someone in a position, just fill-in "vacant".
