The "Form 10" is how DeMolay International reports and keeps track of who is initiated into each chapter. We use this information to update our records for insurance and anything else DeMolay International and DeMolay in Washington might need these legal records for. Washington DeMolay believes in putting chapters first, and having up-to-date information about your Chapter and it's members enables us to do so.

You will need the ID# of your first line signer and all the information from your new members petition so that you can complete the process. Additionally you must have proper access to do so, and a means for processing the payment in order to finish all steps.
